Monthly Award Nominations

Please use this form to nominate individuals and groups for Customer of the Month, Team of the Month, and Direct Support Professional of the Month. The deadline for nominations is the first of each month.

Some characteristics to consider when submitting a nomination include:

  • Significant progress toward a customer’s goals
  • Statistical performance area e.g. turnover, incident reports, utilization
  • Supporting customer through a life circumstance
  • Innovative strategies for positive outcomes
